FDA Issues Public Health Advisory Regarding Levemir Insulin

The U.S. Food and Drug Administration has learned that some stolen vials of the long-acting insulin Levemir made by Novo Nordisk Inc. have reappeared and are being sold in the U.S. market. Three lots or a total of 129,000 vials of this product were stolen in all. These stolen insulin vials may not have been stored and handled properly and may be dangerous for patients to use.

The FDA has received one report of a patient who suffered an adverse event due to poor control of glucose levels after using a vial from one of these three lots.

The agency is advising patients who use Levemir insulin to:

1. Check your personal supply of insulin to determine if you have Levemir insulin from one of the following lots: XZF0036, XZF0037, and XZF0038. Patients can locate the lot number on the side of the box of insulin and also on the side of the vial. Read more...

Researchers Take One Big Step Closer to

One of the fondest hopes of people with type 1 diabetes has long been for the creation of an artificial pancreas, a reliable combination of automated glucose monitoring and insulin delivery that could serve in place of a defunct pancreas.

Apparently the hoped-for day may be here: Researchers from the University of California and Sansum Diabetes Research Institute, in Santa Barbara, California, have demonstrated an automated artificial pancreas system that they say is reliable enough to free type 1 patients from having to manually control their insulin pumps.

The scientists introduced the system, which combines the OmniPod insulin delivery system and the DexCom STS7 continuous glucose monitor with a software component, to the 69th Scientific Sessions of the American Diabetes Association in New Orleans earlier this month. A Call to Allow More People to Undergo Bariatric Surgery

A surgeon who has been at the forefront of exploring bariatric surgery as a potentially curative treatment for type 2 diabetes is calling for it to be made accessible to more people.

Francesco A. Rubino, MD, is the chief of gastrointestinal metabolic surgery at Weill Cornell Medical College and head of the Diabetes Surgery Center at New York Presbyterian/Weill Cornell Medical College.

During a seminar at the recent 69th Scientific Sessions of the American Diabetes Association, he told attendees that the criteria for determining who can undergo the surgery should be changed to allow more overweight people to use it. Read more...

Soy Has No Effect on Men’s Testosterone

A recent meta-analysis that will be published in the journal Fertility and Sterility showed that neither soy foods nor isoflavone supplements from soy affect testosterone levels in men. Researchers looked at more than 50 treatment groups from articles in peer-reviewed journals for this meta-analysis. Soy foods contain phytoestrogens, including isoflavones, which have weak estrogen-like qualities.

Other research has shown that soy consumption decreases the risk of some forms of cancer and heart disease.

Red Meat and Dairy Products Significantly Increase Risk of Pancreatic Cancer

According to a new study, fat from red meat and dairy products is associated with increased risk of pancreatic cancer. As part of the National Institutes of Health-AARP Diet and Health Study, researchers followed and analyzed the diets of more than 525,000 participants to determine whether there is an association between dietary fat and pancreatic cancer. This same study found no association between plant-food fat and pancreatic cancer.

Non-compliance vs. diabetes self care: Are we still playing a blame game?

In 1993, I published an article entitled "Is non-compliance a dirty word?" in The Diabetes Educator in which I expressed my sadness that people with diabetes were actually getting blamed by their health care providers for not following treatment advice (1). I suggested that the patient's failure might really be a failure in the partnership (or lack thereof) between patient and provider.  Fifteen years ago, I challenged diabetes educators to work together with medical practitioners to change noncompliance from a dirty word to a rare occurrence. So how are we doing today?

From non-compliance to self-care

The diabetes lingo has gone from "non-compliance" to "non-adherence" to the inability to perform self-care, all to describe whether or not a patient with diabetes is following their prescribed regimen. No matter what we call it, the outcome is the same. The patient is not doing what we told them to do. The result can be the onset and progression of the complications of diabetes that we all know and dread. The consequences all belong to the patient, not us. My question to my fellow diabetes educators is: Whose regimen is it anyway? Are we taking the time to ask the patient how they choose to manage their diabetes? What are their goals? Have we presented both the benefits and the costs (time, money and energy) of the treatment we are recommending? Does the patient know how to follow their regimen safely and comfortably? Until we do this, the patient is not ready to be independent and perform self-care. Read more...

Broccoli Salad Recipe

This is a nice salad to serve with a summer barbecue. It also makes a wonderful addition to a summer picnic.  Double the recipe to serve more people.

3 cups broccoli florets, chopped
1/2 cup salted sunflower seeds (Optional)
1/4 cup chopped red onion
1/4 cup grated Cheddar, OR Monterey Jack Cheese

Mayonnaise Dressing:
1/2 cup mayonnaise
1/4 cup sour cream
1 tbsp SPLENDA® Granular

Research Disputes FDA Claim that Exenatide Increase Risk of Acute Pancreatitis

In 2008, the U.S. Food and Drug Administration published strong warnings that the type 2 diabetes drug exenatide (trade name Byetta) might increase risk of acute pancreatitis, a painful inflammation of the pancreas. The FDA's action came in the wake of reports that 30 exenatide users had come down with pancreatitis and that six of them had died from the condition.

Now, however, a study published by researchers from Medco Health Solutions, Inc., says that exenatide users run no greater risk of developing pancreatitis than type 2s who take neither drug.

The study results, presented at the recent 69th Scientific Sessions of the American Diabetes Association (ADA), showed that only 0.44 percent of exenatide users experienced an episode of acute pancreatitis. Among sitagliptin users, only 0.28 percent had such episodes. Among the control group of type 2s, however, who had never taken either drug, the rate of incidence was 0.39 percent.

Bobotie (Meat Loaf) recipe

Introduced to South Africa by the Cape Malays, this Indonesian curried meat loaf is to South Africa what Moussaka is to Greece and Lasagne is to Italy.  Traditionally, Bobotie is served with yellow rice (add turmeric), chutney and banana slices dipped in milk.  This is a tasty meat loaf to pack in the cooler for a summer picnic.

2 rye crisp breads OR 2 slices low-carb bread
2/3 cup half-and-half cream
2 lbs ground lamb OR lean ground beef
1 cup diced onion
1 tbsp curry powder
2 tsp salt
1-1/2 tsp ground turmeric
1/2 tsp black pepper
1/4 cup slivered almonds (optional)
2 tbsp seedless raisins, snipped in half
2 tbsp SPLENDA® Granular
1 tbsp lemon juice
4 eggs
4 bay leaves (optional)

In small bowl, break rye crisp breads or low-carb bread in pieces and soak in half-and-half cream. Set aside. In large skillet, fry ground lamb or beef and onion, until meat is browned.  Pour off fat.  Stir in curry powder, salt, turmeric and black pepper. Over small bowl, strain crisp bread and cream mixture; set aside cream.  Stir softened crisp bread or low-carb bread, almonds (if using), raisins, SPLENDA® Granular, lemon juice and 2 eggs into meat mixture until well combined.  Read more...

Some Aspects of Mediterranean Diet More Healthful Than Others

A study in an upcoming issue of the British Medical Journal showed some components of the Mediterranean diet, such as high vegetable consumption and low meat and meat product consumption, are more significantly associated with low risk of mortality than other components, such as cereal consumption and fish consumption. As part of the European Prospective Investigation into Cancer and Nutrition study, researchers followed more than 23,000 Greek men and women for 8.5 years to see how various aspects of a Mediterranean diet affect mortality. Moderate alcohol consumption, high fruit and nut consumption, and high legume consumption were also associated with lower risk of mortality.

Joslin 50-Year Medal Winner Triumphs over Diabetes

Gale Fullerton is a 65-year-old Californian who has the distinction of being a Joslin 50-Year medal winner. Elliott P. Joslin, M.D., knew that good self-management was the key to minimizing long-term diabetes complications, and the medal program was designed as an incentive for those committed to good diabetes care. In 1970, Joslin Diabetes Center expanded the program and began awarding a 50-year bronze medal. They presented the first 75-year medal in 1996.

Since 1970, Joslin Diabetes Center has presented more than 2,632 50-year medals. Joslin has also awarded 23 distinctive 75-year medals from 1996 to the present. More than 636 people have received certificates recognizing 25 years with diabetes.

International Committee Urges Adoption of A1c as the Standard Test for Diagnosing Diabetes

The A1c test (also called the HbA1c test), which establishes average blood sugar levels over a three-month period, should replace fasting plasma glucose and oral glucose tolerance tests as the standard for diagnosing diabetes.

That's the recommendation of an internal committee of experts appointed by the American Diabetes Association, the European Association for the Study of Diabetes, and the International Diabetes Federation. The committee, reporting at the 60th Scientific Sessions of the ADA, offered several reasons for its recommendation:

The A1c test establishes a two- to three-month average that is a more reliable figure than the "snapshot" numbers derived from a fasting plasma glucose or oral glucose tolerance test. The test is easier on patients, who do not have to fast or do anything out of the ordinary other than give a blood sample. The test is more resistant to loss of useful information than either of the glucose tests. For example, if blood samples from the fasting or tolerance tests take too long to reach a laboratory, they lose any measurable glucose. A1c samples, in contrast, retain their measurable constituents. The committee reported that it has set an A1c of 6.5% as the "cut point" for diagnosing diabetes. In other words, individuals with A1c levels of 6.5% and above are considered to have the disease. Individuals with A1c levels running more than 6%, but not quite as high as 6.5%, are considered at high risk of developing type 2. Read more...

AACE Conference Wrap-Up

I attended the annual meeting of the American Association of Clinical Endocrinologists (AACE), held from May 14th to 18th, 2009.  Here's a re-cap of the buzz about ICU glycemic control, prediabetes, and vitamin D.

Inpatient Glycemic Control

An AACE and ADA consensus group called for significant changes in the way that healthcare professionals treat hospitalized patients with high blood glucose levels. They recommended revised glucose targets of 140 to 180 mg/dL in the ICU setting and 100 to 180 mg/dL for most patients admitted to general medical-surgical wards. People were talking about this because of recent attempts to intensively manage glucose targets in the ICU setting. In addition, several recent randomized controlled studies failed to show improvement when trying to achieve near normal glucose levels and instead showed an increase in mortality.  Read more...

I hear voices in my surroundings as the cloud of confusion gradually begins to lift. "Curtis, can you hear me?" "Curtis, what was the score of the football game?" "Curtis, do you know where you are?"

I see the familiar white collars, the short sleeves with darkened crests, the cropped hair, and the bushy moustaches, as the outlines of the paramedics' faces become clearer. Doctors Urged to Stop Accepting Bri- —Oops, Make That Gifts—From Big Pharma

Drug companies spend billions of dollars on research, and it's obvious that they spend more billions on advertising. Well, according to the New York Times, they spend the most billions on giving nice things to doctors: pens, samples, banquets, trips, and educational opportunities among them. For doctors, in fact, there is a free lunch: Pharma companies spend as much as a billion a year just on lunches for doctors. And over 90 percent of doctors have accepted at least some of this largesse from the industry.

But Big Pharma doesn't give away all that stuff without expecting something in return, and what they expect is to influence prescribing behavior. The unspoken assumption is that the doctors will use their nice free pens to write prescriptions for the company's product. Does it work?  Apparently it works a lot better than the physicians themselves realize, because they often underestimate the effect that it does have on them. Social scientists have found that people can't really assess their own bias, especially if that bias is financially beneficial to them. Even free pens exert an influence, and the free lunches are especially effective because people are apparently more receptive to information when enjoying a good meal. Read more...

Foot Amputation Risk in Type 2s Reduced 36 Percent By Blood Fat-Lowering Drug

Patients with type 2 diabetes reduced their risk of having a foot amputated by 36 percent when they took fenofibrate, a drug designed to lower blood fat levels.

An Australian research team at the National Health and Medical Research Council Clinical Trials Centre at the University of Sydney tracked 9,795 type 2s, ages 50 to 75, over a five-year period. Half took 200 mg of fenofibrate daily, while the other half took a placebo. Read more...

Newly Discovered Protein May Give Insight Into Type 2

A previously unknown human protein, called CHC22, may give scientists a powerful new tool for understanding type 2 diabetes, according to researchers from the University of California, San Francisco, and Harvard Medical School.

CHC22 is a form of a so-called "housekeeping" protein called clathrin, which moves other proteins to and fro between cellular compartments. It is an important find because it occurs in humans but not in mice. Because most diabetes research is conducted with laboratory mice, the discovery of a human-only protein that plays a major role in how the body uses glucose is significant. It allows scientists to avoid the obstacles that arise in trying to apply results from research on diabetic animals to type 2 human patients. Read more...
